This is a small website I worked on for one of our internal clients. All functionality, except sending email, is handled on the front-end using jQuery.

Users can add multiple products/services to the request form.
Request Form
Request form uses cookies to detect and add requested services to the form. From here users can remove services from the list, go back to specific service or submit the request.
